FORM 6.0 – INVENTORY AND APPRAISAL. This form from the Stark County, Ohio Probate Court is used by the fiduciary (executor or administrator) to provide a complete and accurate list of all known assets belonging to the decedent’s estate. This form is required under Ohio Revised Code sections 2115.02 and 2115.09 and serves as a foundational step in the probate process. The fiduciary reports the value of assets that can be easily determined, while an appointed appraiser values those assets that are not readily ascertainable. The form categorizes the assets into tangible personal property (such as household items or vehicles), intangible personal property (like bank accounts and investments), and real property (such as land or buildings). It also includes a section to account for automobiles transferred to the surviving spouse under R.C. 2106.18, with a maximum allowable value of $65,000.
