FORM 5.11 - ENTRY GRANTING SUMMARY RELEASE FROM ADMINISTRATION. This form is used in the Probate Court of Stark County, Ohio, to grant a Summary Release from Administration under Ohio Revised Code (R.C.) 2113.031. It allows for the expedited transfer of a decedent’s assets without requiring full estate administration. The court determines that the applicant meets all legal requirements and, as a result, authorizes the direct transfer of the decedent’s personal property, including titled assets, to the applicant. If applicable, the court also issues a Certificate of Transfer to facilitate the legal transfer of real estate.
