FORM 5.1 - ASSETS AND LIABILITIES OF ESTATE TO BE RELIEVED FROM ADMINISTRATION. This form is used in the Probate Court of Stark County, Ohio, to provide a summary of a decedent's assets and debts when requesting relief from full estate administration. This form is typically filed when the estate qualifies for a simplified probate process under R.C. 2113.03, which allows estates with limited assets to be settled more efficiently. The form includes a detailed list of the estate’s assets, such as real estate, automobiles, financial accounts, and personal property, along with their values. Some assets, such as automobiles selected by the surviving spouse under R.C. 2106.18, are excluded from the total asset calculation up to a statutory limit. The form requires the listing of the decedent’s known debts, including creditor names, the nature of each debt, and the total amount owed. If certain assets do not have a readily ascertainable value, an appraiser must assess them, and the appraised values must be indicated on the form.
