FORM 4.1 - SUPPLEMENTAL APPLICATION FOR ANCILLARY ADMINISTRATION. This form is used in the Stark County, Ohio Probate Court when a decedent who lived in another state either owned property in Ohio or had a debtor residing in the county. This form is filed by an applicant seeking authority to administer the Ohio portion of the estate. The applicant must be a resident of Ohio and may qualify under different circumstances. They could be the general executor of the estate already appointed in another state, the executor specifically named in the decedent’s will to handle the Ohio estate, or a local resident applying because the decedent either died without a will or did not designate an Ohio executor. If applicable, an authenticated copy of the decedent’s will from another state must be attached to the application.
