JD-1791T - PERMANENCY HEARING ORDER WITH TERMINATION OF PARENTAL RIGHTS NOTICE. This form is used in the Wisconsin Circuit Court to document findings and legal determinations following a permanency hearing for a child or juvenile placed in out-of-home care. This hearing evaluates whether the child’s current living arrangement continues to be necessary, safe, and appropriate, and whether efforts are being made to return the child home or achieve another permanent outcome, such as adoption or guardianship. The court reviews the agency’s permanency plan, assesses compliance by the involved parties—including parents, service providers, and the child—and determines whether progress has been made toward resolving the conditions that led to the child’s removal from the home. This form also includes a Termination of Parental Rights (TPR) Notice, which serves to inform the parents of the legal grounds that could lead to the involuntary termination of their parental rights. These may include abandonment, failure to assume parental responsibility, child abuse, ongoing need for protection or services, and other statutory reasons.
