JD-1804 - ORDER FOR REVOCATION OF TRIAL REUNIFICATION. This form is used by the Wisconsin Circuit Court to document and decide on a request to revoke a trial reunification for a child or juvenile. It begins by stating whether a Notice and Request for Revocation of Trial Reunification was filed and if there was any objection, or whether a hearing was held on the matter. The court then determines if the trial reunification is still in the best interests of the child or juvenile. Based on these findings, the court orders whether the request to revoke the trial reunification is denied or granted. If granted, the child or juvenile is returned to the previously ordered placement, with the stipulation that this order cannot be used to change placement.
