JD-1783A - STIPULATION TO REVISE DISPOSITIONAL ORDER. This form is used in the Wisconsin Circuit Court to document the agreement of involved parties to make specific revisions to a current dispositional order in a juvenile case without the need for a court hearing. The stipulation cannot be used to request a change of placement or an extension of the order. All parties, including parents, guardians, legal representatives, or other stakeholders, must sign the form to indicate their consent to the proposed changes.
