JD-1788 - ORDER FOR EXTENSION OF DISPOSITIONAL ORDER/CONSENT DECREE (OUT-OF-HOME PLACEMENT ONLY). This form is used in the State of Wisconsin Circuit Court to document a judicial order extending a dispositional order or consent decree for a child or juvenile placed in out-of-home care. It is applicable when the Indian Child Welfare Act does not apply. The form records that a request for extension was submitted, and either no objection was made or a hearing was held. The court evaluates whether the child remains out of the home, whether the request’s allegations were proven, and whether reasonable efforts were made to meet permanency goals and maintain sibling placements when applicable.
