P-1.0641 - NOTICE OF CONFIDENTIAL INFORMATION WITHIN COURT FILING (Contemporaneous Filing). This FLSSI form is used in a probate case in the Circuit Court of a specific county in Florida. It is filed contemporaneously with a document that contains confidential information, as defined by Florida Rule of General Practice and Judicial Administration 2.420(d)(1)(B). The form includes a certification that the filer is submitting a document with confidential information. It requires the filer to specify the title/type of the document and indicate whether the entire document is confidential or if the confidential information is precisely located within the document.
