P-1.0310 - CAVEAT BY INTERESTED PERSON (Other Than Creditor—Nonresident, Pro Se). This FLSSI form is used in the Circuit Court for a specific county in Florida for the purpose of filing a Caveat by an Interested Person in an estate probate case. The form is intended for nonresident caveators who are not represented by an attorney admitted to practice in Florida. The caveator, who has an interest in the estate of the deceased individual named in the form, states their interest and provides information about the deceased's last known residence address, social security number (if known), and year of birth (if known). The caveator also provides their own name, mailing address, and residence address. The caveator designates a member of The Florida Bar, who is a resident of the county, as their agent for the service of notice and consents that service of notice on the agent shall bind the caveator. The caveator requests that the court not admit a will of the decedent to probate or appoint a personal representative without formal notice to caveator or his or her designated agent, and that caveator be given such additional notice as the Florida Probate Rules require. The form includes a declaration under penalties of perjury and provides space for the caveator's and agent's signatures, as well as their addresses and the date.
