Form PC 114 - ORDER FOLLOWING APPEAL OF COMMITMENT FOR TREATMENT OF INFECTIOUS DISEASE. This form is used in Michigan when an individual appeals a prior court order that committed them to a facility for treatment of an infectious disease. After the appeal is filed, a commitment review panel reconvenes, reviews the case, and provides the court with a recommendation on whether the commitment should continue or end.
