Form PC 114 - ORDER FOLLOWING APPEAL OF COMMITMENT FOR TREATMENT OF INFECTIOUS DISEASE. This form is used in Michigan when an individual appeals a prior court order that committed them to a facility for treatment of an infectious disease. After the appeal is filed, a commitment review panel reconvenes, reviews the case, and provides the court with a recommendation on whether the commitment should continue or end. This form allows the judge to record the outcome of that appeal. Based on the panel’s report and the hearing, the court may either deny the appeal and continue the commitment as previously ordered, or grant the appeal and discharge the individual from the treatment facility.
