Form PC 108 - RECOMMENDATION OF COMMITMENT REVIEW PANEL.This form is used in Michigan when a panel of qualified physicians reviews the case of an individual alleged to be, or previously determined to be, a carrier of a serious communicable disease or infection. The panel is composed of medical professionals with training and experience in diagnosing and treating such conditions, and their role is to provide the court with an expert assessment of the individual’s health status and whether they pose a threat to others. Through this form, the panel documents its review of the record, any interviews conducted with the individual, and its findings about whether the person is indeed a carrier and a health risk. The panel also makes recommendations regarding the type of treatment needed and whether the individual should continue to be committed to a facility, be released, or be placed in an alternative program.
