Form PC 107 - ORDER APPOINTING COMMITMENT REVIEW PANEL. This form is used in the Michigan Judicial Circuit – Family Division when the court is considering a petition for the treatment of an infectious disease that may require an individual’s commitment to a health facility. This order establishes a panel of physicians tasked with reviewing the necessity of commitment. The panel must examine the case record, interview the individual (or explain why an interview was not conducted), and provide a written recommendation to the court regarding whether commitment or an alternative course of treatment is appropriate.
