GN-3650 - PETITION FOR TERMINATION OF GUARDIANSHIP OF THE PERSON/ESTATE (ADULT GUARDIANSHIP). This Wisconsin Circuit Court form is used to request that a guardianship of an adult be ended. It allows a ward, guardian, or interested party to petition the court to terminate a guardianship of the person, the estate, or both—typically when the ward is no longer incompetent, has died, moved out of state, or married someone not under guardianship.
