GN-4380 - PETITION TO MODIFY PROTECTIVE PLACEMENT OR PROTECTIVE SERVICES. This form is used in Wisconsin circuit court when there is a need to change the conditions of a court-ordered protective placement or protective services for a ward. This form may be filed by the ward, their guardian, legal counsel, guardian ad litem, the Department of Health Services, the county department responsible for protective services, or another interested party. It allows the petitioner to request changes when the current placement or services are not in the least restrictive environment, are inconsistent with the ward’s needs, or do not align with the resources and obligations of the county department.
