GN-3145 - STATEMENT OF ACTS BY PROPOSED GUARDIAN OF THE ESTATE AND CONSENT TO SERVE (MINOR GUARDIANSHIP). This is a Wisconsin Circuit Court form used in minor guardianship of the estate proceedings. The proposed guardian must disclose any criminal history, bankruptcy filings, and suspensions or revocations of professional licenses, as well as any listing on the Caregiver Misconduct Registry. The form also requires acknowledgment of nomination to serve, agreement to act in the minor’s best interests, and consent to fulfill legal duties, including filing a Guardianship Inventory within 60 days and submitting annual accounts as required.
