GN-4350 - PETITION OBJECTING TO TRANSFER OF PROTECTIVE PLACEMENT. This Wisconsin Circuit Court form is used to object to a proposed or emergency transfer of a person under protective placement. It allows the ward, guardian, attorney, or another interested person to state reasons for objection, such as transfer to an inappropriate or locked facility, lack of a required community plan, or failure to provide the least restrictive or most integrated setting. The petitioner may also claim the transfer is not in the ward’s best interest.
