GN-4170 - PETITION FOR INVOLUNTARY ADMINISTRATION OF PSYCHOTROPIC MEDICATION. This form is used in Wisconsin probate or circuit court when an individual, known as the ward, is under guardianship or protective services and is unable or unwilling to voluntarily take prescribed psychotropic medication. This form may be filed by the Department of Health Services, a county protective services agency, a guardian, or another interested party who has a legal relationship to the ward. The petition provides the court with key details about the ward’s personal information, living situation, legal status, medical history, and treatment needs. It must show that the ward has been prescribed psychotropic medication by a physician, is not competent to refuse treatment, and that attempts to administer medication voluntarily have failed or are not feasible. The petitioner must also demonstrate that without medication, the ward poses a substantial risk of harm to themselves or others, or faces significant deterioration of their mental or physical condition.
