GN-3500 - ACCOUNT OF GUARDIAN/CONSERVATOR (ADULT GUARDIANSHIP AND CONSERVATORSHIP). This form is used in Wisconsin circuit courts to provide a financial accounting of the management of assets and income for an adult under guardianship or conservatorship. The guardian or conservator completes this form to report on financial activity during a specified time period—whether annually, on an interim basis, or as a final report. It includes a summary of beginning balances, new assets and income received, disbursements or losses, and the ending balance of all assets on hand. Detailed schedules (Schedules A, B, and C) must be attached to itemize income, expenses, and current holdings.
