GN-3440 - GUARDIANSHIP OR CONSERVATORSHIP INVENTORY (ADULT GUARDIANSHIP AND CONSERVATORSHIP). This form is used by a guardian or conservator in an adult guardianship or conservatorship case to provide a detailed inventory of the ward’s or individual’s property and assets. It requires the guardian or conservator to list all interests in property, including any marital property interests, regardless of how the assets are titled. The form includes a summary section where the total value of the property is reported, along with any encumbrances or liens, to determine the net value of the estate subject to the guardianship or conservatorship. The guardian or conservator must also attach detailed schedules listing all assets, including digital property, the manner in which the assets are held, any co-owners, and relevant management rights. Additionally, all sources of income for the ward or individual must be separately listed, though income is not included in the summary totals.
