REQUEST FOR MEDIATION. This form is used to initiate a mediation process between parties who are in a dispute. Mediation is a form of alternative dispute resolution in which a neutral third party, the mediator, helps the parties to reach a mutually acceptable resolution to their dispute. The Request for Mediation form typically requires the parties to provide basic information about themselves and their dispute, such as the names and contact information of the parties involved, a brief description of the dispute, and any relevant documentation. The parties must also agree to abide by the AAA's rules and procedures for mediation. Only use this form to file a mediation if there is a contract that provides for mediation by the AAA.
