Request For Arbitration Selection Services (List Only)

REQUEST FOR ARBITRATOR SELECT: LIST ONLY. This form is used by parties who wish to use the American Arbitration Association's (AAA) Arbitrator Select service. This service provides the parties with a list of potential arbitrators from which they can select an arbitrator to hear their dispute. With Arbitrator Select: List Only, the AAA's services are limited to providing the parties with the arbitrator list. The AAA will not handle any other matters related to the arbitration, such as fees charged by the arbitrator, scheduling, or award-related matters. The parties agree that neither the AAA nor any arbitrator listed or appointed under this service shall be liable to any party in any action for damages or injunctive relief for any act or omission in connection with the parties' arbitration or the provision of AAA's Arbitrator Select. If the parties decide to receive full service case administration within six months after utilizing Arbitrator Select, they may file a case under the Standard Commercial Fee Schedule, and they will receive credit for the amount paid for the List Only service. There is a minimum non-refundable search fee of $750 for this service, and if the AAA is unable to compile an appropriate list of arbitrators after completing a search, the balance of the List Only service fees will be refunded. www.FormsWorkflow.com
