REQUEST FOR ARBITRATOR SELECT: LIST AND APPOINTMENT. This form is used by parties involved in a dispute to request the appointment of an arbitrator or a panel of arbitrators through the American Arbitration Association's (AAA) List and Appointment service. The form includes a section on disclosure, which outlines the obligation of any person appointed or to be appointed as an arbitrator to disclose any circumstances that may give rise to justifiable doubt as to their impartiality or independence, including any bias, financial or personal interest in the outcome of the arbitration, or past or present relationships with the parties or their representatives. The AAA is responsible for communicating this information to the parties and the arbitrator, if deemed appropriate. The form also includes a section on the disqualification of an arbitrator, which outlines the criteria for disqualification, including partiality or lack of independence, inability or refusal to perform their duties with diligence or in good faith, or any grounds for disqualification provided by applicable law.
