AOC-E-208 - APPLICATION FOR APPOINTMENT OF GUARDIAN OF THE ESTATE; GUARDIAN OF THE PERSON; GENERAL GUARDIAN FOR A MINOR. This form is used to initiate the legal process of appointing a guardian for a minor child under G.S. 35A-1221 and 35A-1225. This form allows one or more individuals to request appointment as either the guardian of the estate (to manage the minor’s financial assets), guardian of the person (to provide for the minor’s care and living arrangements), or a general guardian (handling both responsibilities). The applicant must provide personal details about the minor, including their age, date of birth, and residence, as well as information about the applicant(s), the minor’s parents (including death certificates if applicable), and any other individuals who may have a legal interest in the proceeding. The form requires disclosure of any existing custody or guardianship orders, or relevant provisions from a parent's will.
