L-1070 - SMALL CLAIMS DECLARATION AND ORDER OF SATISFACTION OF JUDGMENT. This is an Orange County Superior Court form used when a small claims judgment has been fully paid but the judgment creditor has not filed an acknowledgment of satisfaction. The judgment debtor may use this form to declare under penalty of perjury that the judgment, including accrued interest and costs, has been paid in full and that the creditor either refuses to file the satisfaction or cannot be located. Supporting proof of payment, such as canceled checks, endorsed money orders, or signed receipts, must be attached. Approved for Optional Use.
