L-1189 - APPLICATION AND ORDER TO SERVE THE CALIFORNIA SECRETARY OF STATE (Small Claims). This is an Orange County Superior Court form used in small claims cases when a corporation cannot be served through its designated California agent. The form allows a plaintiff to request a court order authorizing service on the California Secretary of State on behalf of a corporation that has no valid agent for service of process, has an agent who resigned, or whose agent cannot be located with reasonable diligence. The form includes a sworn declaration explaining the service issues and a proposed court order for judicial approval.
