L-1091 - CERTIFIED MAIL STATEMENT (SMALL CLAIMS). This is an Orange County Superior Court form used to request service of a small claims action by certified mail through the court clerk. The form explains the risks and limitations of certified mail service, including circumstances where service may be considered invalid, such as unsigned receipts, refusal of delivery, or signatures by unauthorized individuals. Parties may use this form to authorize the Small Claims Division clerk to attempt delivery of the claim by certified mail with return receipt requested.
