This is an Orange County Superior Court form used in California small claims cases when a defendant fails to comply with a payment agreement or stipulated judgment. The form allows the plaintiff to declare that the defendant has defaulted on agreed installment payments or other settlement terms and request entry of judgment for the unpaid balance, including principal, interest, and court costs.
