Description

AOC-CR-220 - NOTICE OF HEARING ON VIOLATION OF UNSUPERVISED PROBATION. This form is used in North Carolina courts to issue a notice of hearing on the violation of unsupervised probation. It is used when there is reason to believe that a defendant who was placed on unsupervised probation has violated the conditions of their probation. The form includes information about the defendant, such as their name, address, race, sex, date of birth, date of judgment, length of sentence, and length of term of unsupervised probation. The form lists the specific violations of probation that the defendant is alleged to have committed. These violations may include failure to make required payments, failure to complete community service, failure to obtain a substance abuse assessment or pay the prescribed fee, failure to participate in a treatment program or pay the prescribed fees, failure to complete an Alcohol and Drug Education Traffic School or pay the prescribed fee, or failure to report to the sheriff as required. The notice informs the defendant that a hearing will be held to determine whether they have violated the conditions of their unsupervised probation. It also states that if the court finds a violation, the probation may be revoked and an active sentence may be imposed. The notice advises the defendant of the potential consequences, such as revocation of their driver's license if they willfully failed to comply with community service requirements. The form includes a section for the date, time, and location of the hearing, as well as a space for the signature of the person issuing the notice. There is also a section for the return of service, where the method of serving the notice is documented. G.S. 15A-1345, 143B-1483. www.FormsWorkflow.com
