DR-425 - DEFAULT APPLICATION - CHILD CUSTODY. This form is used in the Superior Court of Alaska when one parent (the plaintiff) has filed a custody case and the other parent (the defendant) has failed to appear, answer, or otherwise respond within the required timeframe. By completing this form, the plaintiff requests that the court enter a “default,” meaning the defendant loses the right to contest the case because of their lack of response, and asks that the court proceed with granting a default judgment in the plaintiff’s favor based on the custody requests made in the original complaint. The form requires the plaintiff to swear under oath that the defendant was properly served with the summons and complaint, and it includes questions about the defendant’s age, competency, and military status to ensure compliance with legal protections.
