DR-314 - INFORMATION SHEET. This form is used in Alaska family law cases to provide identifying information about the parties and any children involved in the case. This form collects the full legal names, dates of birth, and Social Security numbers of both parents (or parties) and each child connected to the case. The information is used by the court to ensure accurate record-keeping, verify identities, and comply with state and federal laws regarding child support enforcement, including 42 U.S.C. § 666(a)(13). Because it contains sensitive personal details, the form is kept strictly confidential and stored in a secure envelope or file, separate from the public case record. Parties must certify that the information provided is true and accurate, and in uncontested cases, both parties may be required to sign.
