CIV-694 - PETITION TO CHANGE CHILD’S NAME. This form is used in the Alaska Superior Court by a parent or legal guardian who is requesting a legal change of name for a minor child. The form requires the petitioner to provide the child’s current legal name, the new name being requested, and the reasons for seeking the change. It also allows the petitioner to request that the child’s birth certificate be updated, if applicable. The petition must include information about parental consent, such as whether the other parent agrees, whether consent is unnecessary due to death, termination of parental rights, or another reason, and whether supporting documents are attached. The petitioner must also affirm that the request is being made for legitimate personal reasons and not to avoid debts, judgments, or fraud. Once completed and notarized (or self-certified if a notary is unavailable), the petition is filed with the court either through the TrueFiling system or in person/by mail, along with a filing fee or a request for a fee waiver.
