CIV-695 - PARENTAL CONSENT FROM NON-PETITIONING PARENT. This form is used in the Alaska Superior Court during a minor child’s name change case when one parent or legal guardian has petitioned for the change and the other parent or guardian needs to provide their consent. By completing this form, the non-petitioning parent or guardian formally agrees to the child being given the new legal name. The form clarifies that giving consent does not alter the consenting parent’s rights or responsibilities toward the child but acknowledges that, once the court grants the name change, the new name will be legally binding for all purposes and the consent cannot be withdrawn. The parent or guardian also has the option to request or decline notification of future hearings or proceedings in the case. The signed consent must be notarized or affirmed before a court clerk and provided to the petitioner for filing, ensuring the court has a clear record of parental agreement in the name change process.
