CIV-696 - OBJECTION TO CHANGE OF NAME. This form is used in the Alaska Superior Court when a parent, legal guardian, or another party wishes to formally oppose the proposed name change of a minor child. By completing this form, the objecting party explains their relationship to the child and provides reasons for not agreeing to the requested name change. Once filled out, the form must be signed, dated, and a copy served to the petitioner, either through TrueFiling (if available), by mail, hand-delivery, or email (when the petitioner has provided an email address to the court).
