General Form 17 - PETITION-CUSTODY, VISITATION. This is a New York Family Court form used to request an order of custody or visitation. The petitioner—typically a parent, guardian, or relative—must provide identifying information for each child, including name, gender, date of birth, and address, along with details about both the petitioner’s and respondent’s relationship to the child. The form covers essential jurisdictional and factual matters, such as prior or pending custody cases, military service status, existing custody or visitation agreements, and any orders of protection affecting the parties. It also requires disclosure of relevant prior proceedings in New York or other jurisdictions, including Native-American tribunals, and compliance with the Indian Child Welfare Act (ICWA) when applicable.
