Description

General Form 21a - ADDRESS CONFIDENTIALITY ORDER. This form is used in the Family Court of the State of New York. It is issued in response to an affirmation or affidavit filed in the court requesting that a party or child's address(es) and/or other identifying information not be disclosed. The order mandates that the present and all future address(es) and/or other identifying information of the specified individuals should not be disclosed in any pleading, document, or proceeding in the court. The order also designates a person as the agent for service of process and papers in the case. The designated agent could be the Clerk of Court, an attorney, the NYS Department of State, or another specified individual. The protected party is required to inform the designated agent promptly of any changes of address. The order provides information about the time frame for taking an appeal, including deadlines based on receipt in court, service by a party or attorney for the child, or mailing by the Clerk of Court. It also includes a section for the judge or support magistrate to enter the order, and there are checkboxes to indicate whether the order was mailed or received in court, with spaces to specify dates and recipients.
