GF-179 - CONFIDENTIAL PETITION ADDENDUM. This form, filed in the Circuit Court of Wisconsin in the appropriate county, is used in family and paternity cases to provide confidential personal information about the parties and children involved. It collects detailed identifying information for the petitioner or joint petitioner A and the respondent or joint petitioner B, including their full names at birth, dates of birth, social security numbers, and phone numbers.
