GF-182A - PETITION AND AFFIDAVIT CONCERNING SEALING DEFENDANT’S NAME FROM THE COURT RECORD BECAUSE OF IDENTITY THEFT OR MISTAKEN IDENTITY. This form is used by a defendant in a Wisconsin circuit court case to request that their name be sealed from the court record due to identity theft or mistaken identity. The petition must be filed in the county where the case originated and includes the names of the original plaintiff and defendant, as well as the case number.
