GF-153 - ADA ACCOMMODATION REQUEST. This form is used in the Wisconsin Circuit Court to request reasonable accommodations for individuals with disabilities who are participating in court proceedings. The person completing the form may be a party, witness, juror, attorney, or another individual involved in the case who requires support to fully access and engage in the court process.
