Description

NOTICE OF INTENTION TO DISMISS LIEN FOR FAILURE TO APPEAR AT LIEN CONFERENCE OR LIEN TRIAL. This is a California Workers’ Compensation Appeals Board (WCAB) form issued when a lien claimant does not appear at a properly noticed lien conference or lien trial as required under 8 CCR §10888(a). The notice advises the lien claimant that, unless a written objection showing good cause is served on the defendant within the allotted time—15 calendar days, or 20 days if served by mail outside California—the defendant may submit a proposed order requesting dismissal of the lien. The proposed dismissal must include proof of service and a declaration confirming no objection was received. If all procedural requirements are met, the WCAB will issue an order dismissing the lien. www.FormsWorkflow.com
