Lien Conference Deposition Form {WCAB 27}

WCAB Form 27 - LIEN CONFERENCE DISPOSITION FORM. This is a California Division of Workers’ Compensation form used to document outcomes of lien conferences before the Workers’ Compensation Appeals Board. Parties use the form to identify which lien claims have been settled, specifying the lien claimant, lien representative (using UANs), method of resolution—by order or agreement—and the amount paid. The form also records any Notice of Intent to Dismiss Lien Claim issued by the WCAB, as well as lien claimants who withdraw their liens.
