LACIV 150 - REQUEST FOR REFUND. This is a Los Angeles County Superior Court form used to request reimbursement of court fees that were paid in error. The form allows an attorney or self-represented party to specify the amount requested, date of payment, receipt number, depositor information, and the reason for the refund. If the fee was paid through the Court Reservation System (CRS), supporting documentation must be attached showing that the court miscalculated or improperly processed the fee. This form cannot be used to request a refund of jury fees, which requires LACIV 099. The request must be approved by a judicial officer or court manager before a refund is issued.
