DR-210 - AFFIDAVIT OF DILIGENT INQUIRY. This form is used in dissolution of marriage cases when one spouse cannot be located. The petitioner completes this form to document specific efforts made to find the absent spouse before asking the court to allow alternative service, such as publication or other methods. The form prompts for last contact information, last shared address, and detailed search efforts, including letters to family and employers, online and social media searches, court and property record checks, military locator contacts, and any other investigative steps.
