GF-177 - DECLARATION TO SEAL IDENTIFYING INFORMATION IN A CHILD CUSTODY PROCEEDING. This form is used in the State of Wisconsin Circuit Court for cases involving child custody proceedings. It allows a party involved in a custody dispute—where legal custody, physical placement, or visitation is at issue—to request that certain identifying information be sealed from public court records. The purpose of sealing this information is to protect the health, safety, or liberty of the party or the minor child involved, especially when disclosure could pose a risk.
