DR-225 - PROOF OF NOTICE. This form is used in the Superior Court for the State of Alaska in cases where one spouse files a Petition for Dissolution of Marriage and the other spouse cannot be located. It serves as a sworn statement by the petitioner confirming that they have complied with the court’s order regarding notice to the absent spouse. The petitioner affirms, under oath and penalty of perjury, that they have mailed the required documents—typically including the Notice to Absent Spouse and the Petition for Dissolution of Marriage—to the absent spouse’s last known address by both certified and regular mail, as previously ordered by the court. The form must be signed in front of a notary public, court clerk, or other authorized official. If any additional proof of service is required under Alaska Civil Rule 4(e)(6), it must be attached to this affidavit. This form ensures that the court has a verified record that proper notice was attempted or completed before proceeding with the case, thereby satisfying due process requirements.
