MINUTES OF HEARING/ORDER/ORDER AND DECISION ON REQUEST FOR CONTINUANCE/ORDER TAKING OFF CALENDAR/NOTICE OF HEARING. This is a multipurpose California Workers’ Compensation Appeals Board form used by the WCAB to document appearances, procedural events, rulings, and scheduling orders during hearings held in the San Diego district office. The form identifies the case number, type of proceeding (MSC, trial, expedited hearing, lien conference, etc.), party appearances, interpreter information, and any requests for continuances or Orders Taking Off Calendar (OTOC).
