This form is is a court order issued in response to a plaintiff's application under 28 U.S.C. § 1915 to proceed with a civil case without prepaying fees or costs. The order may be granted, granted conditionally, or denied, based on the plaintiff's financial situation. If the order is granted, the clerk is ordered to file the complaint and issue a summons.
