AO 132 (Rev. 12/03), EXEMPLIFICATION CERTIFICATE. This form is used by the United States District Courts (USDC) for the certification of copies of court documents. The first section is a certification by the Clerk of Court that the attached documents are true copies of records of the Court. The clerk affirms that they are the keeper of the records and seal and that the certification is made in accordance with the laws of the United States. The second section is an attestation by a Judicial Officer of the Court that the clerk named in the certification is duly appointed and sworn as the Clerk of Court and keeper of the records and seal. The Judicial Officer also affirms that the certification and attestation are in accordance with the laws of the United States. The third section is a certification by the Clerk of Court that the Judge named in the certification is a duly appointed, sworn, and qualified Judicial Officer of the Court.
