DWC WCAB 30 - SUBPOENA. This form is used in California workers’ compensation cases before the Workers’ Compensation Appeals Board (WCAB) to require a person to appear and testify at a hearing before a workers’ compensation judge. This form is used when testimony is needed but does not require the production of documents (unlike a subpoena duces tecum). It identifies the case number, parties, hearing location, date, and time, and warns that failure to appear may result in contempt penalties. The form includes special requirements for injuries occurring between January 1, 1990 and January 1, 1994, including a mandatory declaration if no Application for Adjudication has been filed.
