Primary Treating Physicians Progress Report {DWC-PR-2}

DWC Form PR-2 - PRIMARY TREATING PHYSICIAN'S PROGRESS REPORT. This form is used in California workers’ compensation cases to provide periodic updates on an injured worker’s medical status. The form is submitted when there is a change in condition, work status, treatment plan, the need for surgery or referral, or in response to a request for information. It captures subjective complaints, objective findings, detailed diagnoses with ICD-10 codes, and a summary of treatment rendered and planned. The PR-2 also documents work restrictions or return-to-work status and requires the physician’s original signature, license number, location, and declaration under penalty of perjury.
